State Sanctions Mental Health Center Where Teens put in Chokeholds, Slapped and Abused

COLUMBUS, Ohio-The Youth Intensive Services center in Youngstown, Ohio, has been ordered by state authorities to halt admissions due to serious deficiencies found during an inspection. A 57-page report identified 47 violations, including instances of inappropriate restraints on teens, such as shoving and extended restraint durations. The facility was also found in disrepair with broken tiles and missing furniture, and it failed to meet staff training and background check requirements. Despite assurances from the center that corrective actions are underway, the state’s action follows a critical report from Disability Rights Ohio, detailing alleged mistreatment and unsafe conditions for children at the facility.
